I'm not sure if I got the topic of your post right...

Although you might not like my answer, I'm guessing that these pictures were photographed professionally.
With distinct lighting on the smoke and some advanced camera settings on a professional set, of which I honestly have no experience at all...

If I were to approach the idea of such a photo, I'd probably let the model literally take a smoke.
However, if that was impossible, I'd try to get a stock image of smoke or take a picture of some candle-smoke against a black background and compose that on top of a picture of your character (in that case you would face some problems at the mouth area, i guess).

If you're asking for detailed information about how to set up your camera I must apologize, for I don`t know how to do that either.

If you are looking fo a tutorial on how to create almost realistic smoke from scratch:
here is one i found using 3dsmax: http://www.3dm3.com/tutorials/smoke/

After going to deviantART and reviewing some photographers galleries that specifically specialize in this particular effect I learned through the comments of the photographers that these particular artist use real smoking models. Some of them used Photoshop to render and/or manipulate the photo, but the base of their photos is from the model exhaling smoke.

My suggestion would be, since smoking is bad for you ;) , try Mr.Kramers tutorial "Frosty Breath".

Maybe you could come up with something and bring that effect to a new level?

Well, yeah... Smoking is bad for you... But there is no harm in taking a drag, holding it back and then exhaling it. So, exhaling real smoke with the right set-up would be the best option, but otherwise I would also do as ChrisAE suggested with the stock image/footgae of smoke against a black background. I can't think of any easy way to create realistic smoke in AE... The particle generators available to AE just don't seem extensive enough to create this kind of effect. Maybe you'd have more luck using a 3D program to create the smoke, such as Cinema 4D which I know has PyroCluster with it, which is able to generate realistic smoke, although I've never seen it used to create wispy smoke like in the example pictures you provided.

not sure if this helps, but if you don't want or your model doesn't want to smoke, you could take your shot, print out a copy, stick it to a wall in a room which you can completely blacken.

Then you get a plastic mask with open lips and position it in some sort of vise so that the lips line up with the photo (your camera will have to be on a tripod of course) then, remove the photo, blacken the room and set up some lighting to cut through the smoke. light a few matches or a cigarette and film away as you put the matches/cig behind the lips so that the smoke passes through them.

Then you can just find the still that you want and composite it in

Does this help? :)

search for some smoke brushes then with the liquify filter and the blending mode you can manipulate it to your desire.

I have an idea of how to make a smoke. I'll try to do it first and I'll share my thoughts with you. Let's just hope it's gonna work.
